What to Expect at Your First Visit
During your initial appointment, we’ll review your medical history, discuss any concerns, and give you time to get to know your doctor. Please plan to arrive a few minutes early so that you can complete any necessary paperwork.
To ensure we provide the best care possible, please bring:
- A valid insurance card
- A list of current prescriptions and over-the-counter medications (with dose and frequency)
- Details about your medical and surgical history
- Any recent test results, x-rays, or other relevant records
Insurance & Payment Information
Family Foot & Ankle Center accepts most major insurance plans. Co-payments and any charges not covered by insurance are the responsibility of the patient at the time of service.
